7. Protected health information

Protected health information includes any individually identifiable health information.

How Used in Healthcare
A disclosure of Protected Health Information is the sharing of that patient health information outside of a covered entity. The sharing of patient health information outside of the health care component or affiliated covered entity is a disclosure. In general, any disclosure of personal health information requires an accounting at the request of the individual, unless the individual gave permission for the disclosure by signing an authorization form, ("Hipaa privacy rule:" 2003).

8. Centers for Medicare & Medicaid Services

The Centers for Medicare & Medicaid Services is an agency that is responsible for administration of several key federal health care programs.

How Used in Healthcare
In addition to Medicare; the federal health insurance program for seniors and Medicaid; the federal needs-based program, CMS oversees the Children’s Health Insurance Program the Health Insurance Portability and Accountability Act and the Clinical Laboratory Improvement Amendments among other services, ("Centers for medicare," 2010).

Cover three basic groups of individual or corporate entities: health plans, health care providers, and health care clearinghouses.
